*In consideration for the time spent completing this questionnaire, each participation will entitle the respondent to an internal voucher with a value of €50, in accordance with the terms specified in the invitation email. This voucher is personal and limited to one per customer. It will be sent to you by email as soon as possible after your participation. It is valid for 90 calendar days from the date of receipt of the email.

This voucher can be used exclusively with Proximus NXT, via its retention manager, for the purchase of a new mobile device or for the subscription to any new fixed connectivity service.

The responses collected as part of this questionnaire are processed by Proximus Luxembourg S.A.
